Installed with some grease and a dead blow hammer. Possibly not tight enough?
I was born with nothing and still have most of it left.

They could be lightly knurled to fit tighter.

If the pipes are mounted correctly, they hang on the bush, and don't exert significant side loads so I don't think they need to be too tight. That said, personally I would not want to be able to put an exhaust bolt in with a small washer under the head and be able to pull the bush out by hand. If you cannot do that, its plenty tight enough.
Same deal with shock mount nuts - folk over tighten them, and they're there to stop side drift only. Tight is fine.
