Spam, unfortunately, is always an issue with discussion forums. The things you need to authenticate users are the same things that keep new users from registering. It is a question of balance which every forum administrator deals with.
This site does pretty well, compared to some I have seen. Ironically, some of the worst forums are run by professionals(?), business support sites for Internet hosts and other communication providers.
